Save & Return address for returning old phone?

I received my new phone and with it two fed ex labels, one I'm assuming is for my old phone return. I'm not sure which to use as there are no explicit instructions. One label is for 'Return centre 1995 North Service rd Oakville' and the other is for 'Shipping 5875 Coopers Mississauga'. I'm assuming the first one but want to be sure, can anyone who has done this confirm?

To send back hardware to our warehouse, the right shipping waybill would be the one with the "Return centre" address, at 1995 North Service rd, Oakville.
